When memory feels slower

For the person who misplaces a word, forgets a name or feels less sharp: a useful first step is to understand what has changed, before shopping for a brain supplement.

Occasional forgetfulness can occur with age, stress or poor sleep. A persistent change, or trouble managing familiar daily tasks, deserves a GP conversation. Nicotine is addictive and is not a sensible self-treatment for memory. Creatine research is interesting but preliminary for cognition. “Nootropic” is a marketing category, not proof of a benefit.

Begin with the pattern

Write down what happens, when it began, whether it is getting worse, and whether it affects medication, bills, cooking, navigation or conversations. Bring a medicines and supplements list to a GP. Sleep problems, mood changes, medication effects, hearing difficulties, thyroid problems and low B12 may contribute and some are treatable. A clinician can decide whether tests or a memory assessment would help.

Sudden confusion, or a new problem with speech, facial droop or one-sided weakness, needs urgent medical help. Gradual worsening that interferes with everyday independence should also be assessed promptly. Occasional retrieval of a name followed by remembering it later is different from repeatedly losing the ability to complete familiar tasks.

Nicotine: an experiment is not a recommendation

Laboratory studies of nicotine have found small, inconsistent short-term effects on attention and some memory tasks. These studies do not establish lasting improvement for an older person with memory concerns. Nicotine can create dependence and has cardiovascular and other adverse effects; smoking and vaping introduce additional harms. Do not start nicotine patches, pouches, vapes or cigarettes for “brain health.” Nicotine replacement has a different purpose when used to stop smoking with appropriate support.

Creatine: promising question, uncertain answer

Creatine has much stronger evidence for muscle strength with resistance training than for memory. Some small trials and pooled analyses suggest cognitive benefits, including in older adults, while reviews disagree on consistency. What about nootropics?

The label can describe caffeine, herbs, vitamins or multi-ingredient blends. Caffeine may improve alertness briefly, but late or excessive use can disrupt sleep and worsen the next day’s clarity. Correcting a confirmed nutrient deficiency is different from taking extra vitamins when levels are adequate. No over-the-counter “brain stack” has been shown to prevent Alzheimer’s disease. Product mixtures may overlap with medicines or each other, so take the actual label to a pharmacist before trying one.

A useful next step for Mum

A simple note over two weeks—examples of forgetfulness, sleep, mood, new medicines, alcohol, and whether daily tasks are affected—can make a GP visit more productive. This is not a diagnosis or a reason to panic. It is a way to take her experience seriously and check for causes that can be addressed.
